What is Te Ara Pae Ora?
Te Ara Pae Ora is a new health and hauora app to support whānau to achieve Pae Ora (Healthy Futures) by providing a pathway to connect to local health, social and community services that can support you in the areas that you need. The app gives you control about the information you want to share with other support people or service providers.
How can Te Ara Pae Ora help me?
- Use Te Ara Pae Ora as a journal to document your Kōrero (what matters to you), so you don’t need to repeat your story to people that you decide to share it with.
- Access information about local organisations that can support your needs.
- Track your progress and watch your ferns grow to Pae Ora – set goals and see progress in different areas of your wellbeing.
- Be a part of your care – create, view, and add notes to care plans with service providers.
- Take charge of your own information – share your kōrero with whānau and support people and see who is accessing your information.
- Keep all of your whānau health and wellbeing information in a safe and secure place.
What is Pae Ora?
Te Ara Pae Ora uses a holistic Māori health concept called Pae Ora, which aims to support all people in Aotearoa to live a healthy flourishing future.
It recognises your health is affected by everything that surrounds you; including your relationships, your values and your connections to our environment.
- Mauri ora is about your individual wellness
- Whānau ora is about the strength of family and connections, and
- Wai ora is about your surroundings
All of these elements are intertwined.
For your life to be as healthy as it can be, use Te Ara Pae Ora to help you weave them together.
Who can access Te Ara Pae Ora?
Te Ara Pae Ora is for anyone who lives in the Kāpiti, Porirua, Wellington and Wairarapa regions.
How do I get started with the app?
Getting started is easy. All you need to register is an e-mail address.
1. Download App from Apple App Store or Google Play or click here to access the tool via the internet.
2. Register using your email address
3. Verify your email address and add your own password to activate your account.
4. Login and start your journey toward Pae Ora.
Which services are involved in Te Ara Pae Ora?
Te Ara Pae Ora will be rolled out to the Kāpiti, Porirua, Wellington and Wairarapa regions. Starting in Kāpiti in November 2023. Service organisations will join the network as we roll out to each region, so the organisations and resources available on Te Ara Pae Ora will grow over time.
Right now, you’ll find links to organisations and information that you can access for information or guidance in the different areas of whānau ora, wāi ora and mauri ora. Check out their websites and contact details for more information about their services.
What if I need urgent help?
Te Ara Pae Ora is not an urgent service. If it is an emergency, ring 111. If you need other urgent help, please contact your healthcare provider, Healthline 0800 611 116 or Plunket Line 0800 933 922. If you need emotional support, please phone or text 1737.
Has privacy been considered?
Āe, keeping your data safe and secure is our priority. Te Ara Pae Ora aims to give you more control of your health and hauora information, so you are aware of who has access and what your information is used for. Te Ara Pae Ora has a dedicated governance group that is manging privacy, security, access, data sharing and service engagement. What happens to the information I put into the app?
The information on Te Ara Pae Ora is private to you and will not be shared unless you invite people to view or you agree to connect to an organisation.
You can share some or all of your Te Ara Pae Ora information with whoever you choose by inviting them, through the app. They can witness your progress, keep you company in activities, or take part in a task that you are unable to do by yourself. You may choose to share parts or all of it with others or provider organisations.
Some information may be used for statistical purposes by a provider as stated in the privacy policy. This data will not identify any personal information. An example of this could be ‘we have worked with 10 people to support them with their goals’. We will share what population data is shared and with who on the website.
What is the Hauora Wellness Plan?
The Hauora Wellness Plan is created by you with your Care Team at your Medical Centre to understand your hauora (Health and Wellbeing) and support you to be as well as you can be. You can access your Hauora Wellness Plan via Te Ara Pae Ora. Contact your Medical Centre to see if they provide Hauora Wellness Plans.
How do I keep my information safe?
Like any app on your mobile device, just remember to not share your password with anyone and log out when you’ve finished updating your information.
At any time you can review and manage who has access to your information from the Care Team section. You can also see who has viewed your information from the audit log from the web portal. Login here to view who has accessed your information.
If you have concerns about any access, Contact help@tearapaeora.org.nz and we’ll help you through the issue.
I'm having trouble logging in
Check you have the correct username and password. Your username can be found in your registration email. You can reset your password by clicking ‘Forgot password’ on the login screen of the app or website. If you need further assistance, contact help@tearapaeora.org.nz
